Dr. Phillip Taggart
Dr. Phillip Taggart earned his medical degree from Oregon Health Sciences University in 1976, completing his internship at LDS Hospital in Utah, with his residency at Good Samaritan Hospital in Portland. Dr. Taggart has practiced Internal Medicine in Eugene for over 30 years. Dr. Taggart is board certified in Internal Medicine and specializes in Adult Primary Care, cardiovascular disease and sports medicine. He is Past President of the Lane Individual Practice Association, previously served as the Internal Medicine Chief at Sacred Heart Medical Center and is a past recipient of the Citizen Life Saving Award. He joins Prime Care Partners after working for the last 18 months in Florence, Oregon.
Dr. Taggart is well known to the community and is looking forward to connecting with past patients as well as new ones.
Dr. Michelle Wyatt
Dr. Michelle Wyatt graduated from medical school at Loyola University Chicago in 1991, completed internship at Rush-Presbyterian St. Luke’s Medical Center in Chicago and then residency at Lutheran General Hospital. She has been Board Certified in Internal Medicine in Oregon since 1994.
She has practiced general Internal Medicine for 23 years, and has interests in wellness/preventative health, prevention of cardiovascular disease, diabetes, and women’s health. Of note, for 5 years in a row, she has been selected as a “Top Doctor” in the Eugene magazine annual survey.
In her free time, Dr. Wyatt enjoys singing, cooking, yoga/meditation, small domestic pets, travel, and Ducks football.
Dr. Doug Jeffrey
A native Oregonian, he spent his first 21 years in Corvallis. He attended OSU and went on to receive his MD from Oregon Health Sciences University. After becoming a specialist with American Academy of Family Medicine in 1981, Dr. Jeffrey served with the United States Public Health Service in Tacoma, Washington for 3 years. He has practiced in the Eugene and Springfield area for over 30 years. As a specialist in Family Medicine, he has enjoyed participating in the primary health care of thousands of Lane County families. Dr. Jeffrey has served on the Board of the Lane Individual Primary Providers (LIPP) for many years. He has directed information technology for LIPP for the last 15 years and been involved in processes to bring health information exchange to Lane County. Of great importance to Dr. Jeffrey has been his Christian faith. Norkenzie Christian Church has kept him busy outside of work. He has a lovely wife and three talented daughters.
Katie Schoonover, FNP
Katie graduated from OHSU’s family nurse practitioner program in 2010. She joins Prime Care Partners after working for several years at Junction City Medical clinic in her family practice.
She feels honored to partner with people in finding ways to get healthy and keep healthy. She especially enjoys the challenge of working with adolescents and individuals with chronic conditions who require a long-term working relationship.
When not at work, Katie loves to run, bike, rock climb and chase her two young boys.
Chelsea Sloanes, PA-C
As early as 16, Chelsea Sloanes became a CNA and then went on to nursing school. She worked as an RN in orthopedics and urgent care for almost 5 years when she decided to pursue becoming a Physician Assistant. She has been practicing in family medicine and urgent care since 2009. Chelsea has a team approach to patient care, with the goal that patients are comfortable with their treatment plan and feel empowered as an active participant in their health. She has lived in the area for 23 years, is married and has a 4 year old son (along with two great danes and a pit bull). She enjoys gardening, a variety of crafting, scuba diving and wedding photography.
Jessica Guido, FNP
Jessica was born and raised on the Oregon coast. She was accepted into OHSU for her undergraduate degree in nursing where she graduated with honors in 2007. From 2007-2013 she was an Intensive Care nurse at PeaceHealth Medical Center in Springfield, OR. In 2008, she moved to Roseburg to marry her best friend Wes Guido. Two beautiful daughters followed, Shaye Beth in 2010 and Devree Chace in 2013. In 2013, she also received her Masters in nursing for Family Practice from Graceland University. She started family practice in Oregon at that time. In September of 2015, she joined the team and started Roseburg PrimeCarePartners. Her ultimate goal is to help her patients optimize their health through exceptional care, understanding, and an open ear.
Jeff Roberts, PA-C
He graduated from the University of Washington School of Medicine and practiced in an internal medicine clinic in Seattle. He has been in practice for 25 years. As Associate Faculty at the School of Medicine, he had the opportunity to work with some of the finest doctors and physician assistants in the country. Eventually, he and his family were drawn back to the Willamette Valley by extended family, friends and fly fishing. For the last 16 years, he has worked in Primary Care and Emergency/Urgent Care medicine. Now Jeff returns to Primary Care in Thurston, to once again establish relationships with patients he has cared for in the past, as well as welcoming new patients into his practice. He adheres to the philosophy that a medical care provider must be a good listener, advocating for realistic goals in achieving a healthy lifestyle. He enjoys caring for patients with both acute and chronic illness, treating people with the respect he would expect for his own family members.
Marissa Reen MS, PA-C
A native of Springfield, Oregon, Marissa always knew she would return to the Pacific Northwest to establish her medical practice. She graduated from Thurston High School in 2008 and received her bachelor’s degree in Biology with an emphasis in pre-medicine at Oregon Institute of Technology in Klamath Falls, Oregon. Marissa pursued her Master’s Degree in Physician Assistant Studies at Western University of Health Sciences in Pomona, California and has been in practice since 2015.
Marissa medical career began as a Certified Nursing Assistant at the age of 16, working in local nursing homes as well as Sacred Heart Medical Center. She developed a fondness for preventative health care, as well as Women’s Health. Her goal is to provide holistic patient care to each and every one of her patients.
In her spare time, Marissa enjoys spending time with her husband and son. She enjoys being outside and exploring new places in the pacific northwest as well as abroad. She is a lifelong fan of the Oregon Ducks and tries to watch as many games as life allows.
Gretta Freyermuth, PA-C
Gretta was born and raised in Pittsburgh, Pennsylvania and complete her undergraduate degree at Mercyhurst University, in Erie Pennsylvania, where she played volleyball and graduated with a degree in Sports Medicine. She received her Master's Degree in Physician Assistant Studies at Lake Erie College, just outside of Cleveland, Ohio. She is Board Ceritified by the National Commission on Certification of Physician Assistants. In her free time she enjoys hiking, running, spinning, all outdoor activities, and cuddling with her new pup. Gretta is accepting new patients of all ages.

John Brandon graduated with a Master of Science in Physician Assistant Studies in 1996 from the University of Utah School of Medicine. John received the Unsung Hero award in 2014 presented by Serenity Lane Health Services Foundation. The unsung hero is a volunteer or professional who has given selflessly to enhance drug and alcohol treatment, awareness, prevention, or education. John is a long term member of the Oregon and National Disaster Medical Team. In his personal time, he enjoys adventures including rafting, bicycling, travel, sports and music.
